Can anyone please help me with the below error:

From the log provided, I can analyse that tP connectivity fails, Perform tp connection check.

Yes your right Shalesh. in STMS it is saying "tp configuration not completely distributed".

can you please help me.

Unfortunately I have deleted STMS while upgrade is running and now trying to configure STMS but it is giving error " The TMS configuration is inconsistent and tp configuration not completely distributed".
